Allow to configure SSO settings for MDM end user authentication (#11270)
Related to #10741, this adds a new key to app config named
`end_user_authentication`, which can be configured using the same keys
as the existing SSO feature.

Per the spec, if the feature is configured, it's implicitly enabled, at
least until we get to #10999.

Note that this only enables the SSO config, a second part of the ticket
with endpoints for the EULA will be tackled separately.

fix flaky TestIntegrationsMDM/TestDEPProfileAssignment (#11304)
This test was failing because we can send other commands when a DEP
device is enrolling (eg: to install a bootstrap package) and the test
was assuming that the first command we'll always get is the command to
install `fleetd`.

We have discussed in the past (@gillespi314 suggested that is important)
increasing the accuracy of the timestamps in the `nano_commadns` table,
which is the proper fix for this.

remove unused logic specific to DEP+Okta+ROP (#11261)
In #10338 we introduced logic to gate DEP profiles behind Okta auth
using the ROP flow.

We're not going to use that, and instead we're going to gate profiles
behind SSO, which can be used from multiple providers and supports SSO
(the initial motivation behind the ROP flow was to create a local user
account.)

This removes some of the old code, which was never used in
production/documented for the public to use.

At the moment I'm leaving the `mdm_idp_accounts` table and related
methods untouched, as it's unclear yet if we're going to need a similar
auxiliar table, and I would rather deal with the migrations all at once.

Remove old mdm_command action (do we really need it?) (#11222)
A question in form of PR:

Do we really need the following two entities in our
[policy.rego](https://github.com/fleetdm/fleet/blob/main/server/authz/policy.rego)
`1. (object=mdm_apple_command, action=read/write)` and `2. (object=host,
action=mdm_command)`? (Maybe mdm_command is a leftover action from the
PoC?)

Guess: `mdm_apple_command` (`fleet.MDMAppleCommandAuthz`) is what we
want: `action=write` means you can enqueue, `action=read` means you can
list commands and read their results.

PS: Found this while trying to add command execution permissions to the
new `GitOps` role.

fix a couple of flaky tests (#11100)
This fixes the following flaky tests:

- `TestHosts/LoadHostByOrbitNodeKey`
- `TestIntegrationsEnterprise/TestListSoftware`
- `TestHosts/ListStatus`

I couldn't figure out what's wrong with `TestScanVulnerabilities` which
is also randomly failing, and it's super slow to run (since it has to
download assets from GitHub) maybe @juan-fdz-hawa can spot it?

### What's happening

- Implemented the hashing mechanism defined by @mna in #10678, however
this mechanism is mainly relevant for batch profile updates via the CLI,
we can't leverage it when a host switches teams.
- Modified `BulkSetPendingMDMAppleHostProfiles` so when two profiles
with the same identifier are sheduled both for removal and update, the
function will now mark only the `install` as `pending` so it's picked by
the cron, and will `DELETE` the `remove` entry from the database so it's
not picked by the cron and never sent to the user.
- `GetHostMDMProfiles` and consequently the profiles returned in `GET
/api/_version_/fleet/hosts` return `host_mdm_apple_profiles.state =
NULL` as "Enforcing (pending", the distinction between `status =
'pending'` and `status IS NULL` is only useful for the cron, for users
both mean the same thing, and all our profile aggregations already
behave this way.
- Using the solution implemented by @gillespi314 in
https://github.com/fleetdm/fleet/pull/10998 we're now deleting the host
row from `host_disk_encryption_keys` if a host is moved from a team that
enforces disk encryption to a team that doesn't.

More robust version handling for macOS software (#10703)
This should get version numbers for more apps on macOS. Notably,
1Password includes helper apps that were getting vulnerability false
positives because we were not picking up the versions.

Addresses #10702.

automatically install a fleetd configuration profile to relevant teams (#10910)
Related to #9459, this adds logic to the cron to add a
`com.fleetdm.fleetd.config` configuration profile to the
`apple_mdm_configuration_profiles` table.

As noted in the comments, this makes some assumptions:

- This profile will be applied to all hosts in the team (or "no team",)
but it will only be used by hosts that have a fleetd installation
without
  an enroll secret and fleet URL (mainly DEP enrolled hosts).
- Once the profile is applied to a team (or "no team",) it's not removed
if
  AppConfig.MDM.AppleBMDefaultTeam changes, this is to preserve existing
agents using the configuration (mainly ServerURL as EnrollSecret is used
  only during enrollment)

Fix disk encryption banner displaying incorrectly on My Device page (#10875)
relates to #10786 

This fixes an issue where users would see the incorrect disk encryption
banners on the my device page. This included a change to the ingestion
logic of the `directIngestDiskEncryptionKeyDarwin` method to take into
account if a host was already being encrypted with filevault locally.

ensure duplicates are removed before enforcing collations (#10814)
Related to #10787, this tries to find in the tables with High likelihood
described in the issue.

This successfully accounts for unique keys that contain leading/trailing
whitespace and are using a collation with a pad attribute set to `NO
PAD` (considers whitespace as any other character instead of ignoring
it)

I haven't found a way to successfully detect the same scenario for
special unicode characters, for example:

Fix SMTP e-mail send when SMTP server has credentials (#10758)
#9609

This PR also fixes #10777.

The issue is: We were using `svc.AppConfig` instead of
`svc.ds.AppConfig` to retrieve the SMTP credentials.
`svc.AppConfig` obfuscates credentials, whereas `svc.ds.AppConfig` does
not.
To help prevent this from happening again I've renamed `svc.AppConfig`
to `svc.AppConfigObfuscated`.
I've also added a new test SMTP server
(https://github.com/axllent/mailpit) that supports Basic Authentication
and tests that make use of it to catch these kind of bugs (the tests are
executed when running `go test` with `MAIL_TEST=1`).

enforce an uniform collation for all tables (#10515)
related to #10441, inspired by the prior work done in
https://github.com/kolide/fleet/pull/1360, this PR:

1. Adds a migration to use `utf8mb4_general_ci` as the default collation
for the database and all the tables. From [MySQL's documentation][1]:

> To change the table default character set and all character columns
    > (CHAR, VARCHAR, TEXT) to a new character set, use a statement like
    > this:
    >
    > ```
    > ALTER TABLE tbl_name CONVERT TO CHARACTER SET charset_name;
    > ```
> The statement also changes the collation of all character columns. If
> you specify no COLLATE clause to indicate which collation to use, the
    > statement uses default collation for the character set.

2. Changes the connection settings to use `utf8mb4_general_ci` as the
default collation, from the [driver docs][2]:

   > Sets the collation used for client-server interaction on
connection. In contrast to charset, collation does not issue additional
queries. If the specified collation is unavailable on the target server,
the connection will fail.

Orbit to set --database_path when invoking osquery to retrieve system info (#10308)
#9132

The actual fix for the empty hosts is adding the `--database_path`
argument in the initial `osqueryd -S` invocation when retrieving the
UUID. Osquery attempts to retrieve the UUID from OS files/APIs, when not
possible (which is what happens on some linux distributions), then it
resorts to generating a new random UUID and storing it in the
`osquery.db`. The issue was Orbit's first invocation of `osqueryd -S`
was not using the same `osquery.db` as the main daemon invocation of
`osqueryd`.

I'm also adding a `hostname` + `platform` to the orbit enroll phase so
that if there are any issues in the future we can avoid the "empty" host
and have some information to help us troubleshoot.

## How to reproduce

On Linux, osquery reads `/sys/class/dmi/id/product_uuid` to load the
hardware UUID.
Some Linux distributions running on specific hardware or container
environments do not have such file available.
The way to reproduce on a Linux VM is to do the following:

# chmod -r /sys/class/dmi/id/product_uuid
```
which will turn the file inaccessible to root.

restore restrictive permissions for GET /mdm/apple and adjust UI (#10282)
Related to #10121 this reverts #10107, and modifies the UI to use
`mdm.enabled_and_configured` instead of the `GET /mdm/apple` endpoint so

Controls page.

add logic to configure FileVault + escrow (#10160)
Related to #9495, this adds the underlying methods to send a
configuration profile that enables FileVault and FileVault Escrow, so we
can fetch and decrypt the encryption key later on.

These methods still need to be called somewhere, and they might need to
be moved outside of `Service`, but at least this gives us a start.

Fix incorrect integer conversion (#10188)
This was caught by CodeQL. We parsed as a 64 bit but then convert to a
(possibly 32 bit) `uint`. It would be 64 bit on most platforms, but we
actually use a 32 bit `int` type in MySQL as well.

Workaround to set policy specs on a team (#9978)
For the CIS benchmark feature, we need a way to import a group of
policies (spec yml) into a team.
This PR adds a flag to `apply -f` to allow setting a team name to a
group of policies.

The "Public IP address" field is sometimes set to a "Private IP" on the
following types of Fleet deployments:
- Local deployments.
- Deployments where Fleet is on a private network.
- Deployments where an agent connects to Fleet not via the public
internet.

This PR will prevent a private IP to be set on the `host.public_ip`
field.
And this PR also adds documentation on how Fleet deduces the public IPs
of the devices so that a user can make the changes to fix this.

This is reproduced in dogfood for `dogfood-centos-box` and
`dogfood-ubuntu-box` where their "Private IP" is also their "Public IP".
Given that these hosts have their "Primary IP" configured to be their
"Public IP" alongside their "Private IP", the `network_interface_unix`
and `network_interface_windows` queries are now changed to ingest only
private IPs for the "Private IP" field.

don't filter DEP hosts by OS before ingesting and improve logs (#9815)
Related to https://github.com/fleetdm/fleet/issues/9653 I couldn't find
any documentation to back this up, but I have a strong suspicion that
the `os` field in the device sync response might come empty in some
scenarios (particularly, when a laptop is brand new, which is hard to
reproduce 😅)

My thoughts are:

1. For the recently purchased MacBooks,
`IngestMDMAppleDevicesFromDEPSync` didn't create an entry in the
database, BUT `nanodep.Assigner.ProcessDeviceResponse` correctly
assigned a DEP profile (the devices were able to enroll). Both methods
filter by `op_type` but only ours filters by `os`.
2. I think this is safe-ish to do, as you will normally assign a MDM
server per device type in ABM

3. I have added extra logs to try to prove this hypothesis next time a
brand new device comes in, let's keep an eye on and re-evaluate this

Add readonly MDM.EnabledAndConfigured to app config and device responses (#9575)
Related to #9571, this adds a new value to both responses which is
calculated when the Fleet server is started, and only set to `true` if
the server is properly configured for MDM.

This helps the UI to determine wether or not we should show certain UI
elements that we only want to show to servers with MDM enabled.

Related to https://github.com/fleetdm/fleet/issues/9588, we now handle 4xx responses from the fleetdm.com server and forward those to the client.

At the time of this commit, the only 4xx response that wasn't already handled by the server is because of an invalid email domain, so we assume that, but we should look into establishing a pattern of error messages with the website instead.

Run authorization check before processing policy spec (#9270)
I was getting a cryptic error and no logs in fleet when trying to apply
an invalid spec file:

```

With the changes in this PR now I get a more descriptive error (bad
request):

**Use nano's multi service**

This allows us to integrate more seamlessly with nano and to run our
custom MDM logic _after_ the request was handled by nano, which gives us
more flexibility (for example: now we can issue commands after a
TokenUpdate message)

From nano's code:

> MultiService executes multiple services for the same service calls.
> The first service returns values or errors to the caller. We give the
> first service a chance to alter any 'core' request data (say, the
> Enrollment ID) by waiting for it to finish then we run the remaining
> services' calls in parallel.

**Integration tests + fixes**

- Move some of the service logic from `cmd/` to `server/service`
- Add integration tests for the MDM enrollment flow, including SCEP
authentication.
- Fixed a bug that set `host_mdm.mdm_id = 0`  during MDM enrollment due
  to how MySQL reports the last insert id when `ON DUPLICATE KEY` is
  used.
- Completely remove the host row from `host_mdm` when a device is
  unenrolled from MDM to match the behavior of how we ingest MDM data
  from osquery

The value of this attribute is used by macOS in "System Preferences >
Profiles" to display a note saying "This Mac is supervised and managed
by <xyz>".

When <PayloadOrganization> is not set, it uses the URL of the MDM server
for the message, which looks unpolished.

fix issue with duplicate vulns detected using nvd (#8613)
The OVAL analyzer falsely assumes that any vulnerabilities detected on a
host only come from OVAL. However, it is possible that NVD detects
vulnerabilities on these hosts even though it excludes software from
deb_packages and rpm_packages. For example, a python package twisted
v22.20 has a vulnerability CVE-2022-39348 detected by NVD. The OVAL
analyzer would delete this vulnerability, and it would be re-inserted by
the NVD scanner on the next run. This creates a loop.

The fix is to only delete vulnerabilities that are actually detected
using OVAL. We already store this in the source column in the
software_cve table.

improve primary ip host detail query (#8407)
Previously, the primary ip was determined using the interface with the most traffic. This had a few issues and required explicitly filtering out certain interfaces that we don't want eg loopback, docker.

Instead, use the default route to determine the primary (private) ip. If there are multiple, use the one with the lowest metric and prefer ipv4 addresses.

This abstracts the default config we use to run the server into a
function so it can be used in tests to run an HTTP server using the same
configuration.

Additionally, this fixes a data race in tests, as an HTTP server
configuration can't be changed once you call `server.Start()`[1]

fix a bug causing features to be false (#8240)
Related to https://github.com/fleetdm/fleet/issues/8010 and https://github.com/fleetdm/fleet/issues/8013 this prevents a bug that happens when:

1. A team doesn't have a `config.features` key in the JSON stored in the table or `config` is `NULL`
2. The team is edited from the UI

All `config.features` will default to `false`, which can be a problem if your global settings are `true` for both (which is the default)

add browser-related security headers to HTML responses (#8180)
related to #8031, this adds the following headers to HTML responses:

- Strict-Transport-Security: informs browsers that the site should only
  be accessed using HTTPS, and that any future attempts to access it
  using HTTP should automatically be converted to HTTPS.
- X-Frames-Options: disallows embedding the UI in other sites via
  <frame>, <iframe>, <embed> or <object>, which can prevent attacks like
  clickjacking.
- X-Content-Type-Options: prevents browsers from trying to guess the MIME
  type which can cause browsers to transform non-executable content into
  executable content.
- Referrer-Policy: prevents leaking the origin of the referrer in the
  Referer.

additionally, this ensures we set `X-Content-Type-Options` for CSV and
installer responses.

add headers denoting capabilities between fleet server / desktop / orbit (#7833)
This adds a new mechanism to allow us to handle compatibility issues between Orbit, Fleet Server and Fleet Desktop.

The general idea is to _always_ send a custom header of the form:

fleet-capabilities-header = "X-Fleet-Capabilities:" capabilities
capabilities              = capability * (,)
capability                = string
```

Both from the server to the clients (Orbit, Fleet Desktop) and vice-versa. For an example, see: 8c0bbdd291

Also, the following applies:

- Backwards compat: if the header is not present, assume that orbit/fleet doesn't have the capability
- The current capabilities endpoint will be removed

### Motivation

This solution is trying to solve the following problems:

- We have three independent processes communicating with each other (Fleet Desktop, Orbit and Fleet Server). Each process can be updated independently, and therefore we need a way for each process to know what features are supported by its peers.
- We originally implemented a dedicated API endpoint in the server that returned a list of the capabilities (or "features") enabled, we found this, and any other server-only solution (like API versioning) to be insufficient because:
  - There are cases in which the server also needs to know which features are supported by its clients
  - Clients needed to poll for changes to detect if the capabilities supported by the server change, by sending the capabilities on each request we have a much cleaner way to handling different responses.
- We are also introducing an unauthenticated endpoint to get the server features, this gives us flexibility if we need to implement different authentication mechanisms, and was one of the pitfalls of the first implementation.

add a mechanism to deprecate AppConfig settings w/ backwards-compat (#7353)
Related to https://github.com/fleetdm/fleet/issues/7312, the motivation behind these changes is to introduce a way to deprecate configurations in `AppConfig`, while still preserving backwards compatibility.

From the Epic:

> NOTE: `host_settings` is now replaced by `features`. We should still support `host_settings` as an alias to `features` for backwards compatibility, but in our communications, we should use and recommend features as the canonical way forward.

Bug with Vendor Width migration (#7339)
Updated migration used to increate the width of 'software.vendor' to
address issue found when running migration in aurora: "ALGORITHM=INPLACE is not
supported. Reason: cannot silently convert NULL values, as required in
this SQL_MODE. Try ALGORITHM=COPY."
